Manifestações da proteinose alveolar: descrição de um caso e revisão da literatura
Resumo
a proteinose alveolar pulmonar (PAP) é uma doença rara que afeta o interstício pulmonar de forma difusa, resultando no acúmulo de surfactante pulmonar e obstrução alveolar, o que pode levar à insuficiência respiratória. Neste artigo, descrevemos o caso de um paciente do sexo masculino, com 32 anos de idade, residente em Bogotá, Colômbia. O paciente foi admitido devido a um agravamento progressivo da classe funcional, necessitando de oxigênio suplementar. Os achados no exame físico incluíam estertores finos bilaterais, além de acropaquias. Uma tomografia computadorizada (TC) de tórax foi realizada, revelando um “padrão empedrado”. Além disso, um estudo broncoscópico foi conduzido, com citologia do lavado broncoalveolar com um conteúdo proteico abundante, bem como macrófagos com coloração de PAS (ácido periódico de Schiff) positiva e anticorpos anti-GM-CSF
positivos. Com base nesses achados, foi feito o diagnóstico de proteinose alveolar, e posteriormente,
foi realizado um lavado pulmonar total. O paciente apresentou melhora na oxigenação e, portanto,
recebeu alta hospitalar. Dado o escasso número de estudos sobre PAP na Colômbia, optou-se por
descrever um caso e revisar a literatura.
